Community-Based Prevention, Intervention, Reunification

 

Service Areas: Rio Arriba, Taos, Los Alamos, Santa Fe, and Bernalillo Counties 

CBPIR serves caregivers of children ages 0–18 with models designed for prevention and reunification. Each model; FSS, FC, FRS and TLR has specific eligibility criteria and timelines to meet the diverse needs of families.  

Service Location: CBPIR services are provided in the home.

FSS

Family Support Services

Services typically last up to six months, focusing on prevention and short-term stabilization. 

• Referrals accepted through CYFD investigations, community partners, or self-referrals. 

• Provides case management, an evidence-based parenting curriculum (Strengthening Families), and wraparound meetings with community providers to coordinate care and strengthen family supports. 

• Supports may include: housing, utilities, food assistance, transportation, education, medical, dental, vision, and home-visiting resources. 

FC

Family Connections

Services generally extend up to one year, depending on family progress and identified needs

• Referrals accepted from CYFD investigations (self-referrals also accepted). 

• Uses the Family Connections curriculum and CASI (evaluation-of-change survey) to identify needs and create a Service Plan. 

• Focuses on strengthening family stability, safety, and long-term wellbeing. 

FRS

Family Reunification Services

Services Up to 6 Months After Trial Home Visit

• Begins 2–3 months prior to trial home visits and may continue up to six months afterward. 

• Referrals accepted from CYFD Permanency Caseworkers following investigation closure. 

• Provides stabilization and follow-up support to promote permanency and safety for children returning home. 

• Includes evidence-based parenting curriculum and wraparound meetings with community providers to support family success and community connections.  

TLR

Time-Limited Reunification

Services may be provided for up to one year following the date the child(ren) begin a trial home visit, regardless of when the referral was received by the contractor. 

• Referrals accepted from CYFD Permanency Caseworkers. 

• Provides intensive case management, evidence-based parenting curriculum, and wraparound meeting services with community providers to ensure coordinated family support. 

• A comprehensive family assessment identifies needs and measurable goals to ensure child safety and stability.
