Prevention & Support Services-Reflection of Tier Services
Las Cumbres offers a continuum of family-centered prevention and support services that empower families to build on their strengths, enhance protective factors, and create safe, nurturing environments where children can thrive. Our programs use evidence-based approaches that promote resilience, stability, and overall family well-being.

Family Outreach (FO)
Offers short-term, solution-focused case management (up to three months) for families identified as low-risk or in need of minimal support. The program helps families connect families with community resources, and early intervention services to promote stability.
Community-Based Prevention (CBP)
The Community-Based Prevention & Intervention (CBP) program provides in-home support for caregivers of children ages 0–18 across five Northern New Mexico counties, focusing on prevention, stabilization, and reunification. Through four tailored models—FSS, IFSS, TLR, and FRS—families receive case management, parenting education, and wraparound services to strengthen safety, stability, and long-term well-being. Referrals are accepted from CYFD, community partners, or families themselves, depending on the model.
Keeping Families Together (KFT)
Program combines stable, affordable housing with intensive, tiered case management to support homeless or precariously housed families with children under 18 who are involved or at risk of involvement with CYFD. Services include rental assistance, parenting education, health and employment referrals, and legal advocacy—all designed to promote family stability, prevent foster care placement, and reduce child abuse and neglect.
