Family Infant Toddler Program & Early Intervention (FIT/EI)

Conjunto Therapeutic Preschool
At Conjunto, students of different capabilities learn alongside one another: approximately one-third have developmental disabilities or delays; one-third have emotional or psychological special needs, and one-third can be considered “mainstream.” Contact 505.901.1119

Our therapists partner with children ages prenatal to 6 and their families to create a support plan specific to their circumstances, values, and culture. The clinical team is part of the National Child Traumatic Stress Network (NCTSN) and provides evidence-based therapies to address childhood trauma. Meeting times and locations are flexible.

Family Outreach
Grandparents Raising Grandchildren (GRG)
Provides specialized support, resources, and discussion opportunities for grandparents who are serving as primary caregivers. Contact 505.753.4123
Family Support and Prevention Services
Family-centered case management services focused on prevention and support. These programs empower families to build on their strengths, enhance protective factors, and create safe, nurturing environments where children can thrive. Contact 505.795.4505
Offers short-term, solution-focused case management (up to three months) for families identified as low-risk or in need of minimal support. The program helps connect families with community resources, and early intervention services to promote stability.
Community-Based Prevention, Intervention, Reunification (CBPIR)
Provides in-home support to families in Santa Fe, Rio Arriba, Los Alamos, Taos, and Bernalillo counties, with a focus on prevention, stabilization, and reunification. Through four tailored case management models, families receive parenting education and wraparound services to strengthen safety, stability, and long-term well-being.
Keeping Families Together (KFT)
Provides intensive case management services for up to two years, for insecurely housed families living in Bernalillo County who are involved or at risk of involvement with Child Protective Services. KFT combines housing assistance with a wide range of supports to ensure family stabilization, prevent foster care placement, and reduce child abuse and neglect.

Around-the-clock services, making it possible for adults with development disabilities, whose needs are beyond the scope of parents or family members, to live in a home setting.
Adult Respite
Qualified providers offer respite services to parents, family members, and guardians of adults ages 21+ who have developmental delays or disabilities. Serving residents in Los Alamos, Rio Arriba, & Santa Fe Counties.
